Hardship Waiver Request
Please fill out the form below to request a hardship waiver of your late fee or of your full 2026 ASHA membership dues. Full dues waivers will only be granted in cases of extreme and unexpected hardship. Waivers may be considered in the following cases and with appropriate supporting documentation:
- Medical hardship (examples of supporting documentation include doctors note, FML paperwork)
- Financial hardship (examples of supporting documentation include collections notice, unemployment claim)
- Natural Disaster (examples of supporting documentation include insurance claims, photos of home damage, notice of work closures due to natural disaster)
Waivers will not be granted for voluntary breaks from the workforce. Refunds will not be granted. If you have received a waiver in previous years, you must provide supporting documentation to indicate your hardship circumstances are current/ongoing.
